Lichen planus treatment in Gurgaon

Lichen planus is itchy, stubborn and often leaves dark marks on Indian skin, but it is very treatable. Dermatologist Dr. Tanya Kochhar confirms the diagnosis, looks for medicines or infections that may be driving it, and treats the itch, the active rash and the pigmentation as separate stages.

Quick answer

How is lichen planus treated in Gurgaon?

Lichen planus treatment in Gurgaon starts with a dermatologist confirming the diagnosis, often with dermoscopy and sometimes a small biopsy, then calming the rash with strong steroid creams, antihistamines for itch and, for widespread disease, tablets or phototherapy. Skin lichen planus usually settles within one to two years. The dark marks it leaves fade more slowly and are treated separately.

Understanding lichen planus

What lichen planus is

Lichen planus is an inflammatory skin condition in which the immune system attacks the lowest layer of the skin or the lining of the mouth. On the skin it causes very itchy, shiny, flat-topped bumps, usually purple on fair skin and dark brown or almost black on Indian skin. They favour the wrists, ankles, shins, lower back and genitals, and often join into plaques. DermNet, the New Zealand dermatology reference, estimates that it affects about 1% of people, mostly adults aged 30 to 60.

Indian data match what dermatologists see in clinic. A 2025 study in Cureus of 50 patients at Subharti Medical College, Meerut, found classical lichen planus in 52%, the thick hypertrophic type in 16%, a sudden widespread eruptive type in 12%, sun-exposed actinic lichen planus in 8% and lichen planus pigmentosus in 6%. Itch was severe in 68%. Under the dermatoscope, the fine white lines called Wickham striae were seen in 23 of the 26 classical cases, which is why a dermoscopy examination often settles the diagnosis without a biopsy.

Lichen planus is not contagious, not caused by poor hygiene and not a sign of cancer. Most skin lichen planus clears completely. The parts that need watching are long-standing sores in the mouth or genitals, which are reviewed regularly.

Types of lichen planus and how each behaves

The same condition looks very different depending on where it appears, and the treatment changes with it.

Classical skin lichen planus

Itchy, flat, purple-brown bumps on the wrists, shins and back. Usually settles within one to two years, often leaving dark marks.

Hypertrophic lichen planus

Thick, warty, intensely itchy plaques on the shins. It lasts longer than the classical type and often needs injections into the plaque.

Lichen planus pigmentosus

Grey-brown or slate patches on the face, neck and skin folds, with little or no itch. Common in Indian skin and easily mistaken for melasma.

Oral lichen planus

Lacy white lines on the inner cheeks, or red, sore areas that sting with spicy food. Often long-lasting and followed up with your dentist.

Scalp (lichen planopilaris)

Redness, scaling and itch around hair follicles that can destroy them and leave permanent bald patches. It needs prompt treatment.

Nails and genitals

Ridged, thinned or split nails, and white or eroded patches on the genitals. Both are treated early to prevent permanent damage.

Side by side

Is it lichen planus, psoriasis, eczema or melasma?

Many people arrive having used creams for the wrong condition. These are the clues a dermatologist uses to tell lichen planus from its common look-alikes.

Lichen planusCommon look-alikes
SurfaceShiny, flat-topped bumps with fine white lines (Wickham striae).Psoriasis: thick silvery scale. Eczema: dry, rough, oozing or crusted skin.
Colour on Indian skinViolet to dark brown or near black bumps.Psoriasis: pink-red to brown plaques. Eczema: ill-defined dark, thickened patches.
Usual sitesWrists, ankles, shins, lower back, mouth and genitals.Psoriasis: elbows, knees, scalp. Eczema: elbow and knee creases, hands, face.
Face and neck patchesLichen planus pigmentosus: grey-brown, often in folds, may start with mild itch.Melasma: brown, symmetrical on the cheeks and forehead, no itch, darkens with sun.
MouthLacy white lines on both inner cheeks.Oral thrush wipes off. Tobacco patches (leukoplakia) are usually solid white and need a separate check.
Medicine linkA lichen planus-like rash (lichenoid drug eruption) can follow some blood pressure tablets, antimalarials and others.Other drug rashes are usually more sudden, widespread and red.

When the picture is unclear, a small skin biopsy under local anaesthetic gives a definite answer. It is worth it, because steroid creams that suit lichen planus can make a fungal infection worse, and melasma treatments do little for lichen planus pigmentosus.

Why it happens

What causes lichen planus?

The exact cause is not known. The British Association of Dermatologists describes it as an abnormal immune response, and lists several things that are linked to it in some people: viral infections such as hepatitis C and hepatitis B, medicines including some blood pressure tablets, antimalarials and antidepressants, metals such as mercury in dental fillings for mouth lichen planus, and stress.

A rash caused by a medicine is called a lichenoid drug eruption. It looks very like lichen planus, and the key step is to identify the likely medicine and, with your prescribing doctor, decide whether it can be changed. Never stop a prescribed medicine on your own. The rash fades slowly after the trigger is removed, often over weeks to months.

Lichen planus also tends to appear along scratches, cuts and surgical scars, which is known as the Koebner response. That is one reason why controlling itch early matters: scratching spreads the rash into new lines and deepens the dark marks it leaves.

Bring a list of every medicine and supplement you have taken in the last year, including ones you have stopped. It is one of the most useful things you can do at the first visit.

At Le Dermaire

Lichen planus treatment in Gurgaon, step by step

Treatment follows a ladder. Each step is added only if the one before is not enough, and the dark marks are dealt with once the active rash is quiet.

1

Confirm the diagnosis

Dr. Tanya examines the skin, mouth, scalp and nails, uses dermoscopy to look for Wickham striae, and suggests a small biopsy if the picture is not clear.

2

Look for a driver

Your medicines are reviewed for a possible drug cause, and blood tests such as hepatitis C screening are advised where appropriate.

3

Calm the itch and rash

Strong steroid creams or ointments, gentler non-steroid creams for the face and folds, antihistamines at night, and injections into thick plaques. Tablets or phototherapy are added for widespread disease.

4

Treat the marks and follow up

Once no new bumps appear, sun protection and pigment-fading creams start. Mouth, scalp and genital lichen planus are reviewed at regular intervals.

Treatment options

Lichen planus treatment options explained

Which option is used depends on how widespread the rash is, where it is, and your general health. Most people need only the first two or three.

Steroid creams and ointments

Potent topical steroids are the first-line treatment for skin lichen planus and settle itch and inflammation in most people. They are strong medicines, used for a set period on specific areas and reviewed, because long unsupervised use thins the skin.

Tacrolimus and pimecrolimus

Non-steroid anti-inflammatory creams that suit the face, neck, armpits, groin and lichen planus pigmentosus, where long steroid use would thin the skin. They can sting for the first few days.

Injections into thick plaques

Hypertrophic lichen planus on the shins and stubborn patches are injected with a small amount of steroid directly into the plaque, which reaches deeper than any cream. The procedure is explained on the scar treatment page.

Tablets for widespread disease

A short course of oral steroids may be used for a severe, widespread eruption. For long-lasting disease, other tablets such as acitretin, methotrexate or hydroxychloroquine may be considered, each with its own blood test monitoring and not all suitable in pregnancy.

Narrowband UVB phototherapy

Controlled ultraviolet light treatment, given two to three times a week, is an option for widespread skin lichen planus that does not settle on creams. It needs regular visits over several weeks.

Treating the dark marks

Post-inflammatory pigmentation and lichen planus pigmentosus are treated with strict sun protection, tacrolimus and gentle depigmenting creams. Lasers and peels are used only with great caution, if at all.

Setting expectations

How long does lichen planus last?

Classical lichen planus of the skin usually clears within one to two years, and in most people it does not come back, although a minority have repeat episodes months or years apart. Treatment does not shorten the underlying course dramatically, but it controls the itch within weeks, stops new bumps forming and limits the damage that scratching causes. Thick hypertrophic plaques on the shins can last for years without treatment.

The dark marks are the part most people find hardest. Once the bumps flatten, they often leave brown to grey-black patches, especially on Indian skin, and these can take many months or several years to fade. Starting sun protection early and avoiding further irritation makes a real difference. Mouth, scalp, nail and genital lichen planus tend to be long-lasting and are managed as long-term conditions.

For more detail on how scalp involvement is assessed, see the hair and scalp treatment page; nail changes are covered under nail treatment.

Long-standing sores or thickened white patches in the mouth or genitals carry a small risk of developing into skin cancer over many years. Regular review, stopping tobacco and gutkha, and reporting any lump, ulcer or patch that changes are the safeguards.

Lichen planus treatment cost in Gurgaon

Most people are treated with a consultation, a prescription plan and follow-up visits. The overall cost depends on how widespread the condition is, whether a biopsy, blood tests, injections or phototherapy are needed, and how long the follow-up lasts. Anything beyond the consultation is explained and quoted before it is done.

Why the cheapest fix often costs more

Mixed steroid and antifungal creams bought over the counter can quiet lichen planus for a while, then thin the skin, spread a fungal infection or leave the dark marks deeper. A diagnosis and a short, supervised plan usually works out cheaper than months of trial and error.

Everyday care

Living with lichen planus

Helps

 • Using the prescribed cream exactly as directed, on the areas shown, for the time advised.
 • A thick, fragrance-free moisturiser after bathing to ease itch and dryness.
 • Broad-spectrum sunscreen on the face and neck every day, especially with lichen planus pigmentosus.
 • Soft toothbrush and a mild, non-foaming toothpaste if the mouth is affected.

 • Scratching, scrubbing or loofahs, which spread the rash along new lines.
 • Over-the-counter mixed steroid and antifungal creams used without a diagnosis.
 • Very hot, spicy or acidic food, alcohol and tobacco if you have mouth lichen planus.
 • Bleaching creams, aggressive peels or laser packages for the dark marks while the rash is still active.
 • Stopping a prescribed medicine on your own because you suspect it caused the rash.

Why a dermatologist

Lichen planus care led by a dermatologist

Lichen planus is a medical diagnosis, not a cosmetic one. A dermatologist can tell it apart from psoriasis, eczema, fungal infection and melasma, decide when a biopsy is worth doing, review your medicines with your other doctors, and prescribe and monitor the strong creams, injections and tablets it sometimes needs. Dr. Tanya also plans the pigmentation stage carefully, because the wrong laser or peel can make lichen planus marks darker. Lichen planus is one of the conditions covered under general dermatology at Le Dermaire.

Dr. Tanya Kochhar holds an MBBS from Kasturba Medical College, Manipal, an MD in Dermatology, Venereology and Leprosy from MGM Medical College, a DNB, and a post-doctoral fellowship in Aesthetic Dermatology (FRGUHS). She is registered with the National Medical Commission and the Delhi Medical Council.

Common questions

Lichen planus treatment FAQs

Is lichen planus curable?

Skin lichen planus usually clears on its own within one to two years, and in most people it does not return, so in that sense it resolves. Treatment controls the itch and stops new bumps while that happens. Mouth, scalp, nail and genital lichen planus are often long-lasting and are controlled rather than cured.

Is lichen planus contagious?

No. Lichen planus is an immune reaction in the skin, not an infection, so it cannot be passed on by touch, sharing towels or sexual contact. It also does not run strongly in families. Family members do not need to be treated.

What is the fastest way to stop lichen planus itching?

A potent prescription steroid cream or ointment on the bumps, a thick moisturiser and a sedating antihistamine at night usually ease the itch within two to four weeks. Thick plaques respond faster to an injection. Avoid scratching, which spreads the rash and deepens the dark marks.

How do I get rid of lichen planus dark marks?

Wait until no new bumps are appearing, then use daily sunscreen and the creams your dermatologist advises, such as tacrolimus or gentle depigmenting agents. The marks fade slowly, over months to years. Strong peels, bleaching creams and aggressive lasers can irritate the skin and make the pigmentation darker.

Can lichen planus be caused by medicines?

Yes. Some blood pressure medicines, antimalarials such as hydroxychloroquine, diuretics and others can cause a lichen planus-like rash called a lichenoid drug eruption. Your dermatologist reviews your medicines and, with your prescribing doctor, decides whether one can be changed. Do not stop a prescribed medicine without advice.

Is lichen planus related to hepatitis C?

Hepatitis C has been linked to lichen planus, particularly the mouth type, in a number of studies, and the strength of the link varies between countries. Many dermatologists therefore advise a hepatitis C blood test, especially for oral or widespread lichen planus. A positive result is treated by a physician and may help the skin as well.

Is oral lichen planus dangerous?

Most oral lichen planus is a nuisance rather than a danger, causing lacy white lines or soreness with spicy food. Long-standing red or ulcerated areas carry a small long-term risk of mouth cancer, so they are reviewed regularly and any patch that changes is biopsied. Stopping tobacco and gutkha lowers the risk further.

How much does lichen planus treatment cost in Gurgaon?

Most people need only a consultation, prescription treatment and follow-up visits. Costs rise if a biopsy, blood tests, injections or phototherapy are needed, and these are explained before they are done. The consultation fee at Le Dermaire is Rs 1,800.
