Melasma treatment in Gurgaon

Melasma can be controlled well, but it rarely responds to shortcuts. Dermatologist Dr. Tanya Kochhar builds a step-by-step plan around sun and light protection, prescription creams and, where suitable, gentle procedures, with maintenance planned from the start.

Quick answer

How is melasma treated in Gurgaon?

Melasma is treated with daily tinted sunscreen, prescription creams and, where suitable, gentle peels or low-energy laser alongside them. At Le Dermaire, Dr. Tanya Kochhar builds a step-by-step plan with maintenance from the start. Most people see gradual lightening within 8 to 12 weeks. Melasma is controlled rather than cured, as it tends to return.

Understanding melasma

What melasma is

Melasma is a common pattern of pigmentation that appears as symmetrical brown or grey-brown patches on the cheeks, forehead, bridge of the nose, upper lip or chin. It is much more frequent in women and in medium to darker skin, including most Indian skin types.

It is not simply a tan or a set of sun spots. Melasma involves overactive pigment cells, changes in the blood vessels of the skin and damage to its lower layers from long-term light exposure. That is why it tends to come back, and why a treatment that strips the skin quickly so often makes it worse.

Melasma is controlled rather than cured. Most people can get their patches much lighter and keep them that way, but it takes steady daily care and a willingness to avoid aggressive treatments that promise fast clearing.

What sets it off

Common melasma triggers

Most people have more than one trigger. Identifying yours is part of the consultation.

Sun and UV

The strongest trigger. Even short, daily exposure on a commute or balcony is enough to darken melasma.

Visible light

Light from the sun, and to a lesser extent screens and indoor lighting, can deepen melasma in darker skin. Tinted sunscreens containing iron oxides help block it.

Hormones

Pregnancy, oral contraceptives and hormone therapy commonly trigger or worsen it. Thyroid problems are sometimes linked and may be checked.

Heat

Kitchen heat, saunas, hot yoga and hot weather can darken melasma even without direct sun.

Irritation

Harsh products, scrubs, waxing and inflamed skin all encourage pigment cells to make more melanin.

Genetics

Melasma often runs in families, which is one reason it can persist despite good care.

At Le Dermaire

How melasma is treated

A treatment ladder: each step is added only if the one before is not enough.

1

Diagnosis

Dr. Tanya confirms that the patches are melasma rather than another kind of pigmentation, uses dermoscopy to judge whether the pigment is epidermal, dermal or mixed, and reviews your triggers and products.

2

Photoprotection

A broad-spectrum tinted sunscreen, the right amount and reapplication, plus shade and a hat. Without this step, nothing else holds.

3

Prescription topicals

Creams are chosen for your skin and adjusted every few weeks. Oral tranexamic acid may be considered for some people after a medical screen.

4

Procedures & maintenance

Superficial peels or low-energy laser may be added for stubborn patches. Once the skin is even, a lighter maintenance routine keeps it that way.

Treatment options

Melasma treatment options explained

Topical agents do most of the work. A hydroquinone combination, sometimes with a retinoid and a mild steroid, is effective but is used for a limited period under supervision, because long-term use can cause irritation and a rare bluish-grey darkening called ochronosis. Azelaic acid, topical tranexamic acid, niacinamide and similar agents are gentler options for longer use and maintenance.

Tranexamic acid by mouth has good evidence in moderate to stubborn melasma. It is a prescription medicine that is not suitable for everyone, particularly people with a history of blood clots, and it is only started after a medical history and screening.

Chemical peels, such as glycolic or lactic acid at superficial strength, can speed up fading once the skin has been prepared with topicals. Laser is used cautiously in melasma. Low-energy Q-switched or pico laser may help selected patients, but aggressive laser can cause rebound darkening and, with repeated sessions, patchy white spots (mottled hypopigmentation) that are very hard to reverse.

Why fast clearing is a warning sign

Melasma packages that promise quick results often rely on aggressive laser or strong steroid creams. Both can lighten patches for a while and then leave them darker, or cause white spots and thinning that cost far more to manage. A slower plan that keeps working is better value.

Setting expectations

What to expect from melasma treatment

Epidermal melasma usually responds best, often lightening noticeably within two to three months. Dermal and mixed melasma fade more slowly, and some pigment may remain even with good care. Improvement is rarely a straight line: patches can darken after a holiday, a hot summer or a change in hormones, and the plan is adjusted when that happens.

Melasma that appears in pregnancy sometimes fades on its own within a year of delivery. During pregnancy and breastfeeding, treatment is limited to sun protection and a few agents considered safe, such as azelaic acid, and most other creams, oral tranexamic acid and procedures are postponed.

If you started melasma after beginning an oral contraceptive or hormone therapy, do not stop it on your own. Discuss it with the doctor who prescribed it, since alternatives may be available.

Everyday care

Living with melasma

Helps

  • Tinted broad-spectrum sunscreen every morning, including at home and on cloudy days.
  • Reapplying every 2 to 3 hours outdoors, or a powder or stick sunscreen over makeup.
  • A wide-brimmed hat and shade, especially in the middle of the day.
  • A gentle cleanser and moisturiser that do not sting or irritate.

Avoid

  • Steroid creams or fairness creams used without a doctor's advice.
  • Scrubs, peel-off masks and anything that leaves the skin red.
  • Waxing the face and upper lip, which can inflame the skin.
  • Saunas, steam and prolonged heat where you can.

Why a dermatologist

Melasma care led by a dermatologist

Melasma is easy to mistake for other pigmentation, and easy to make worse. A dermatologist can confirm the diagnosis, judge the depth of the pigment, prescribe and monitor medicines such as hydroquinone combinations and tranexamic acid safely, and decide whether a peel or laser is likely to help or harm your particular skin.

Dr. Tanya Kochhar holds an MBBS from Kasturba Medical College, Manipal, an MD in Dermatology, Venereology and Leprosy from MGM Medical College, a DNB, and a post-doctoral fellowship in Aesthetic Dermatology (FRGUHS). She is registered with the National Medical Commission and the Delhi Medical Council.

Common questions

Melasma treatment FAQs

Can melasma be cured permanently?

Melasma is a long-term condition that is controlled rather than cured. With daily sun and light protection, prescription creams and sometimes gentle procedures, most people can lighten their patches considerably. It tends to return with sun, heat or hormonal change, so a lighter maintenance routine is continued after the patches have faded.

How long does melasma take to fade?

Most people see gradual lightening within 8 to 12 weeks of consistent treatment. Epidermal melasma usually responds faster than dermal or mixed melasma, which can take several months and may not clear completely. Stopping sunscreen or treatment early is the most common reason melasma comes back.

How much does melasma treatment cost in Gurgaon?

At Le Dermaire, many patients are treated with a prescription routine and follow-up visits. If peels or laser are recommended, the cost depends on the number of sessions and is quoted in writing after the consultation, before booking. The consultation fee is Rs 1,800, and nothing is charged without a quote first.

Is laser good for melasma?

Laser is not a first-line treatment for melasma. Low-energy Q-switched or pico laser may help selected patients alongside creams and sun protection. Aggressive or frequent laser can cause rebound darkening and permanent white spots, especially in Indian skin, so it is used cautiously and only after the skin has been prepared.

Is tranexamic acid safe for melasma?

Tranexamic acid, as a cream or by mouth, is widely used for melasma. Oral tranexamic acid is a prescription medicine and is not suitable for people with a history of blood clots, certain heart conditions, or during pregnancy. It is only started after a medical history and screening, and it is monitored during treatment.

Why does my melasma come back?

The underlying tendency remains even when the patches fade. Sun and visible light, heat, hormonal changes such as pregnancy or contraceptives, and irritating products can all reactivate the pigment cells. Daily tinted sunscreen and a maintenance routine reduce relapses, and flare-ups are treated early before they deepen.

Can melasma be treated during pregnancy?

Treatment in pregnancy is limited to careful sun protection, a mineral tinted sunscreen and a few agents considered safe, such as azelaic acid. Hydroquinone, retinoids, oral tranexamic acid, peels and laser are generally postponed. Pregnancy melasma often lightens after delivery, and fuller treatment can start once you are no longer breastfeeding.
